Leflunomide works by inhibiting the mitochondrial enzyme dihydroorotate dehydrogenase (DHODH), which is essential for de novo pyrimidine synthesis. Activated lymphocytes require rapid DNA synthesis for proliferation. By blocking pyrimidine production, leflunomide suppresses the expansion of autoreactive T and B lymphocytes, thereby reducing immune-mediated inflammation.
