Vildamark-M Tablet is a fixed‑dose oral anti‑diabetic combination containing Metformin hydrochloride (a biguanide) and Vildagliptin (a dipeptidyl peptidase‑4 [DPP‑4] inhibitor). It is prescribed for the management of type 2 diabetes mellitus in adults, especially when Metformin alone does not provide adequate glycaemic control or when combination therapy is clinically appropriate.

Metformin lowers blood glucose by reducing hepatic glucose production, improving insulin sensitivity, and enhancing peripheral glucose uptake. Vildagliptin inhibits the DPP‑4 enzyme, prolonging incretin activity, which increases insulin secretion and decreases glucagon release in a glucose‑dependent manner. Vildamark-M Tablet works through two different paths. Metformin acts as an "insulin sensitiser"; it stops the liver from overproducing glucose and helps the muscles and fat cells absorb sugar from the blood more easily. Vildagliptin acts as a "DPP-4 inhibitor"; it protects the body's natural incretin hormones from being broken down. These hormones signal the pancreas to release more insulin when you eat and prevent the liver from releasing stored sugar into the blood. Together, they lower blood sugar levels throughout the day and night.

• Always carry a source of fast-acting sugar (like glucose tablets or candy) in case you experience symptoms of low blood sugar.

• Your doctor will likely order regular blood tests to check your kidney function and liver enzymes while you are on this medication.

• Inform any doctor or dentist treating you that you are taking metformin, especially if you are scheduled for surgery or a scan involving contrast dye.

• Stay well-hydrated, especially during illnesses involving vomiting or diarrhoea, to prevent kidney strain.

• Keep the tablets in their original blister packaging until you are ready to take them.

• Do not use after the expiry date.

• Food: Taking the medication with food is recommended to minimize gastrointestinal discomfort.
• Alcohol (SEVERE): Significantly increases the risk of lactic acidosis and can cause unpredictable changes in blood sugar levels.

• ACE Inhibitors / NSAIDs (CAUTION): These medicines may affect kidney clearance and increase Metformin exposure. Your doctor may check kidney function more closely if these are used together.

• Hepatotoxic Drugs (CAUTION): Vildagliptin has been linked to rare cases of liver changes. If used with other medicines that affect the liver, your doctor may recommend periodic liver function tests.

• Heart Failure (CAUTION): Use with care in patients with unstable heart failure.

• Dehydration (CAUTION): Increases the risk of kidney issues while taking Metformin.

Taking more than the prescribed dose of this medicine can be harmful. In rare cases, it may lead to low blood sugar or a condition called lactic acidosis. Signs to watch for include unusual tiredness, rapid breathing, or changes in skin color. If an overdose is suspected, seek emergency medical help immediately.

If you miss a dose, take it as soon as you remember with food. If it is almost time for your next dose, skip the missed one and continue with your regular schedule. Do not take a double dose to make up for a missed one.
