NULL is an antiplatelet medicine (commonly known as a blood thinner) used to reduce the risk of heart attack and stroke. It contains Clopidogrel, which works by preventing platelets in your blood from sticking together and forming harmful blood clots. This medicine is typically prescribed to patients with a history of heart disease, recent stroke, or peripheral arterial disease. By keeping the blood flowing smoothly through the arteries, NULL helps prevent serious cardiovascular events and is often a critical part of long-term cardiac care.

NULL is a thienopyridine-class antiplatelet medication used for the secondary prevention of atherothrombotic events. It contains Clopidogrel, a prodrug that must be metabolised by the liver into its active form to be effective. It is indicated for adults who have experienced a recent myocardial infarction (heart attack), recent ischaemic stroke, or established peripheral arterial disease.

The medication works by selectively inhibiting the binding of adenosine diphosphate (ADP) to its platelet P2Y12 receptor and the subsequent ADP-mediated activation of the glycoprotein GPIIb/IIIa complex. By doing so, NULL irreversibly inhibits platelet aggregation for the remainder of the platelet's lifespan (approximately 7–10 days). This prevents the formation of thrombi (clots) in "hardened" or narrowed blood vessels (arteries), a process known as atherothrombosis, which can lead to life-threatening events like a stroke or heart attack.

NULL is often prescribed as a standalone therapy or in combination with low-dose aspirin (dual antiplatelet therapy), depending on the patient's specific cardiovascular risk profile, such as after the placement of a coronary stent. Regular adherence is vital, as the protective effect against clot formation depends on maintaining steady levels of the medication in the bloodstream.

To prevent another cardiovascular event in patients who have recently suffered a myocardial infarction.

To reduce the risk of recurrence in patients with a history of stroke or transient ischaemic attack (TIA).

To improve circulation and reduce the risk of clot-related complications in patients with established artery disease in the limbs.

Acute Coronary Syndrome (ACS):

Management of unstable angina or heart attack, often in combination with aspirin.

Post-Stenting:

To prevent stent thrombosis after a coronary artery bypass or angioplasty.

Serious Side Effects (Seek urgent medical attention if you experience):

• Gastrointestinal Bleeding: Blood in your stools, black/tarry stools, or vomiting blood.
• Haemorrhagic Stroke: Sudden severe headache, weakness on one side of the body, or slurred speech.
• Thrombotic Thrombocytopenic Purpura (TTP): A rare condition involving fever, bruising, and yellowing of the skin/eyes (jaundice).
• Severe Allergic Reaction: Swelling of the face, lips, or throat; difficulty breathing.

• Dosage: The standard dose is one 75 mg tablet once daily. Take it exactly as prescribed by your doctor.
• Administration: Swallow the tablet whole with a glass of water. It can be taken with or without food.
• Consistency: Take your dose at the same time every day to maintain a consistent antiplatelet effect.
• Duration: Do not stop taking this medicine unless your doctor tells you to, even if you feel well. Stopping abruptly can significantly increase your risk of a heart attack or stroke.
• Missed Dose: If you forget a dose, take it as soon as you remember. If it has been more than 12 hours, skip the missed dose and take the next one at the usual time. Never double the dose.

NULL is an inhibitor of platelet activation and aggregation through the irreversible binding of its active metabolite to the P2Y12 class of ADP receptors on platelets.

1. Metabolic Activation: Clopidogrel is a prodrug. Once ingested, it is metabolised by the cytochrome P450 enzymes (mainly CYP2C19) in the liver into its active thiol metabolite.
2. Receptor Inhibition: The active metabolite binds permanently to the P2Y12 receptors on the surface of platelets.
3. Prevention of Aggregation: This binding prevents Adenosine Diphosphate (ADP) from activating the platelets. Since ADP is a primary signal for platelets to stick together (aggregation), blocking its receptor ensures that platelets remain "slippery" and do not form a clot (thrombus) inside the artery.
4. Irreversible Effect: Because the binding is irreversible, the affected platelets are inhibited for their entire lifespan (roughly 10 days), providing long-lasting protection.

• Bleeding Precautions: Use a soft toothbrush and an electric shaver to minimize the risk of bleeding from minor injuries.
• Identify Yourself: Carry a medical alert card stating that you are taking an antiplatelet medication.
• Surgery Prep: Always inform your dentist or surgeon that you are on NULL before any procedure.
• Watch for Bruising: It is normal to bruise more easily, but report large or unexplained bruises to your doctor.

• Store NULL in its original packaging at room temperature, ideally between 20°C and 25°C (68°F to 77°F).
• Keep the medicine away from excessive heat, moisture, and direct sunlight.
• Always keep the tablets out of the reach of children and pets.
• Do not use the medicine after the expiry date printed on the packaging.

Grapefruit Juice: Avoid consuming large amounts of grapefruit or its juice, as it may interfere with the liver enzymes (CYP2C19) that activate Clopidogrel, potentially making the medicine less effective.

• Proton Pump Inhibitors (PPIs): Drugs like Omeprazole or Esomeprazole can significantly reduce the effectiveness of Clopidogrel. Use Pantoprazole or Rabeprazole if an antacid is required.
• NSAIDs: Taking ibuprofen, naproxen, or high-dose aspirin with NULL increases the risk of stomach ulcers and bleeding.
• Other Anticoagulants: Using Warfarin, Heparin, or Rivaroxaban concurrently significantly increases bleeding risks.
• SSRIs: Certain antidepressants (like Fluoxetine or Sertraline) may increase the risk of bleeding.

• Active Bleeding: Contraindicated in patients with active pathological bleeding, such as a peptic ulcer or intracranial haemorrhage.
• Bleeding Disorders: Use with caution in patients with conditions that increase the risk of bleeding (e.g., haemophilia).
• CYP2C19 Poor Metabolizers: Some people have a genetic variation that makes them less able to convert Clopidogrel to its active form, reducing its effectiveness.

The usual daily dose of Clopidogrel is 75 mg taken once daily by mouth. It can be taken with or without food and is best taken at the same time each day to maintain a consistent effect. The exact dose and duration of treatment should always be followed as prescribed by a healthcare professional.

An overdose may lead to prolonged bleeding time and subsequent bleeding complications. If an overdose is suspected, seek emergency medical attention immediately. There is no specific antidote; treatment is symptomatic and may involve platelet transfusion.

Take it as soon as you remember. If it is nearly time for the next dose, skip the missed one. Never take two doses together.
