NULL is a prescription antihypertensive medication containing of Olmesartan Medoxomil. It belongs to a class of drugs known as Angiotensin II Receptor Blockers (ARBs). It is primarily used to treat high blood pressure (hypertension) in adults and children. Relaxed blood vessels facilitate blood flow and allow the heart to pump more efficiently, thereby reducing the risk of severe cardiovascular events.

NULL is an oral medication composed of Olmesartan Medoxomil, a prodrug that is hydrolysed to its active form, olmesartan, during absorption from the gastrointestinal tract. It is a cornerstone therapy for the long-term management of essential hypertension.

Composition and Mechanism: Each tablet contains of Olmesartan Medoxomil. The medication works by selectively blocking the binding of angiotensin II (AT). This potent hormone causes blood vessels to constrict through activation of AT receptors in vascular smooth muscle and the adrenal gland. By preventing this binding, NULL promotes vasodilation (widening of the blood vessels) and reduces the secretion of aldosterone, which helps the body eliminate excess salt and water.

Major Therapeutic Uses: The primary use of NULL is the treatment of hypertension. It can be used alone or in combination with other classes of antihypertensive agents, such as diuretics or calcium channel blockers. Reducing high blood pressure is essential for decreasing the incidence of fatal and non-fatal cardiovascular events, primarily strokes and myocardial infarctions (heart attacks).

Benefits and Precautions: A significant advantage of Olmesartan is its potent and long-lasting blood pressure-lowering effect, often remaining effective for a full 24 hours. Unlike some older antihypertensive drugs, it typically does not cause a dry cough. However, precautions are necessary regarding kidney function and potassium levels. A rare but serious condition called sprue-like enteropathy (severe chronic diarrhoea with weight loss) has been associated with Olmesartan, sometimes appearing months or years after starting the drug.

Medical Supervision and Risks: NULL must be used under the supervision of a physician. Regular monitoring of blood pressure, kidney function (creatinine), and serum potassium levels is standard. The most significant risk associated with this class of drugs is fetal toxicity; therefore, it must be discontinued immediately if pregnancy is detected.

• Bronchitis or Pharyngitis: Symptoms similar to a common cold or sinus infection.

• Nausea: Occasional stomach upset.

Uncommon Side Effects:

• Hyperkalemia: Elevated levels of potassium in the blood.

• Vertigo: A sensation of spinning.

• Rash: Mild skin irritation or itching.

• Fatigue: General feeling of tiredness.

Serious Side Effects (Requires immediate medical attention):

• Sprue-like Enteropathy: Severe, chronic diarrhoea with significant weight loss.

• Angioedema: Swelling of the face, lips, tongue, or throat that can cause difficulty breathing.

• Kidney Dysfunction: Significant decrease in urine output or swelling in the legs and ankles.

• Severe Hypotension: Fainting or extreme lightheadedness (dangerously low blood pressure).

• Timing: Take the tablet once daily, preferably at the same time each day (e.g., every morning).
• Method: Swallow the tablet whole with a glass of water. It can be taken with or without food.
• Consistency: For best results, it is essential not to miss doses, even if you feel fine. Hypertension often has no visible symptoms.
• Do Not Stop Abruptly: Stopping your blood pressure medication suddenly can cause your blood pressure to rise rapidly (rebound hypertension).

Olmesartan works through a "Receptor-Blocking" mechanism:

1. Prodrug Conversion: After oral administration, the body converts medoxomil to the active metabolite olmesartan.
2. AT Receptor Blockade: It specifically binds to the Angiotensin II type 1 (AT) receptors on the surface of blood vessels.
3. Vasodilation: By blocking the hormone angiotensin II from "docking" at these receptors, the vessels are prevented from tightening. This causes the vessels to widen (relax).
4. Salt/Water Balance: It also reduces aldosterone secretion, which helps the kidneys excrete excess water and sodium, thereby lowering blood pressure.

• Rise Slowly: To prevent dizziness or fainting, get out of bed or stand up from a chair slowly.

• Monitor Potassium: Avoid using potassium-based salt substitutes or potassium supplements unless told otherwise by your doctor.

• Check the Scale: If you develop severe, watery diarrhoea and lose weight rapidly, stop the medication and call your doctor immediately.

• Home Monitoring: Keep a log of your blood pressure readings at home to share with your doctor.

Store NULL at room temperature (20°C to 25°C). Keep the tablets in their original container, tightly closed, to protect them from moisture and light. Keep out of reach of children.

• Salt Substitutes: Many salt substitutes contain potassium instead of sodium. Using these while on NULL can lead to dangerously high blood potassium levels.

• Grapefruit Juice: Unlike some other heart medications, Olmesartan does not have a significant interaction with grapefruit juice.

NSAIDs (e.g., Ibuprofen)

ACE Inhibitors

CAUTION

Generally, it is not recommended to use both together, as it increases the risk of kidney damage.

• Renal Artery Stenosis: Use with extreme caution, as it may cause acute kidney failure in these patients.

• Diabetes (taking Aliskiren): Do not use NULL in combination with aliskiren if you have diabetes.

• Dehydration: If you have severe vomiting or diarrhoea, your blood pressure could drop too low while on this medication.

Your doctor determines the dose based on your blood pressure response.

• Standard Starting Dose: once daily.

• Maintenance Dose: If after 2 weeks, blood pressure is not controlled, the dose may be increased to 40mg once daily.

• Pediatric Dosing: For children, the dose is usually calculated based on body weight.

• Doctor's Direction: Do not change your dose without consulting your physician.

If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed one and return to your regular schedule. Do not take two tablets at once to catch up, as this can cause your blood pressure to drop too low.
