NULL is a prescription medicine classified as a Xanthine Oxidase Inhibitor. Uric acid is a waste product normally produced by the body during the breakdown of purines (substances found in certain foods and body tissues). When the body produces too much uric acid or the kidneys cannot remove it efficiently, it builds up in the blood, a condition known as hyperuricemia. This excess can lead to the formation of needle-like crystals in the joints (causing gout) or stones in the urinary tract.

NULL works by interfering with the chemical process that creates uric acid. It inhibits the enzyme xanthine oxidase, which is responsible for converting precursor chemicals into uric acid. By lowering these levels, the medication allows existing crystals to slowly dissolve and prevents new ones from forming. It is important to note that NULL does not relieve a gout attack that has already started; in fact, uric acid levels shifting at the start of treatment can sometimes trigger a temporary flare. For this reason, it is often started alongside low-dose anti-inflammatory medicines. Because it is processed and removed by the kidneys, regular monitoring of renal function is vital to ensure the dose remains safe and effective.

Gout Prophylaxis

Tumor Lysis Syndrome

Serious Side Effects (Seek Urgent Medical Care)

• Severe Skin Rash: Any rash, redness, or peeling skin (could be a sign of a rare but life-threatening reaction called SJS or DRESS).

• Liver Issues: Yellowing of eyes/skin, dark urine, or persistent loss of appetite.

• Kidney Changes: Change in the amount or color of urine, or painful urination.

• Blood Disorders: Fever, sore throat, easy bruising, or unusual bleeding.

• Severe Allergic Reaction: Swelling of the face, tongue, or throat and difficulty breathing.

NULL should be taken exactly as prescribed. It is best taken after a meal to reduce stomach irritation. You must drink plenty of fluids (at least 2 liters or 8–10 full glasses of water a day) while taking this medicine to help prevent kidney stones and assist the kidneys in flushing out uric acid. If you are taking a single daily dose, try to take it at the same time each day. Do not stop taking the medicine even if you have a gout flare, as your doctor will provide separate medications to manage the pain while NULL continues to work on your uric acid levels.

NULL works by reducing the production of uric acid in the body through the following process:

• Enzyme Inhibition: It blocks the action of an enzyme called xanthine oxidase. This enzyme is the primary catalyst that turns hypoxanthine and xanthine into uric acid.

• Lowering Blood Levels: By stopping this conversion, NULL reduces the concentration of uric acid in the blood and urine.

By lowering the overall uric acid "load" in the body, NULL prevents the crystallization of urate in joints and tissues, thereby stopping the inflammatory cycle of gout and the formation of kidney stones.

• Watch for Rash: A skin rash is the most important warning sign. If you see any spots, itching, or redness, stop the medication and call your doctor immediately.

• Stay Hydrated: Drinking plenty of water is not optional; it is a critical part of the treatment to protect your kidneys.

• Initial Flares: Don't be discouraged if you have a gout attack shortly after starting. This happens as crystals move; it means the medicine is working to shift the uric acid.

• Dietary Changes: Continue to limit high-purine foods like red meat, organ meats, and shellfish to help the medication work better.

• High-Purine Foods: Consuming excessive red meat, seafood, and sugary fructose drinks can counteract the uric acid-lowering effects of the medicine.

• Vitamin C (High Dose): Large doses of Vitamin C may increase the risk of kidney stones when combined with allopurinol.

• Azathioprine / Mercaptopurine (Severe): NULL dramatically increases the levels of these drugs, which can lead to life-threatening bone marrow toxicity. Doses must be reduced significantly.

• Amoxicillin / Ampicillin (High): Increased risk of developing a skin rash when taken together.

• Warfarin (Moderate): May slightly increase the effect of blood thinners, requiring more frequent blood clotting tests.

• Diuretics (Moderate): Water pills can increase the risk of allopurinol hypersensitivity, especially in those with kidney issues.

• Renal Impairment (Severe): Requires strict dosage reduction to prevent severe skin reactions and toxicity.

• Bone Marrow Suppression (High): Patients with existing blood disorders require close monitoring.

• Hypertension (Moderate): Certain blood pressure medications can increase the risk of allergic reactions to allopurinol.

If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular schedule. Do not take a double dose to make up for a missed one.
